Community Smiles Dental is a nonprofit dental clinic dedicated to providing affordable pediatric dental care and oral health services for underserved children and families. We serve low-income patients, pregnant women, and individuals with special healthcare needs, delivering compassionate care that helps build healthy smiles and stronger communities.

If you’re interested in our services, the first step is knowing whether your family is eligible. You can use this checklist to quickly see if you qualify for our services.
I have BadgerCare, Forward Health, or Medicaid insurance
I am uninsured and have income that falls below 200% of the Federal Poverty Guidelines.
**Patient priority is provided to youth age 18 and younger. For patients without dental insurance, a discounted fee/copay is required.
